| 10% | Deep acne affecting less than 40 percent of the face and neck, or; deep acne other than on the face and neck |
| 0% | Superficial acne (comedones, papules, pustules, superficial cysts) of any extent |

Step by step
- File an Intent to File (Form 21-0966) to lock your effective date.
