ED secondary to mental health (worked example)

ED secondary to PTSD, anxiety, or depression is the most common secondary claim in VA practice — and one of the highest-yield because it unlocks SMC-K regardless of the schedular rating.

The medical basis

There are three independent medical pathways linking mental health to ED:

  1. The psychophysiology of the stress response affects arousal.
  2. The medications used to treat PTSD/anxiety/depression (SSRIs, SNRIs, benzodiazepines) cause ED as a documented side effect.
  3. The relationship dysfunction and avoidance behaviors associated with these conditions affect sexual function.

Any one of the three is sufficient for nexus.

A 2015 Journal of Sexual Medicine review study found that 85% of male combat veterans with PTSD reported ED, compared to 22% of male combat veterans without a mental health diagnosis.

What you file

  1. Medical diagnosis of ED in a medical record (from your primary care physician, urologist, or VA provider)
  2. Confirmation that your primary mental health condition is already service-connected at 0% or higher
  3. Nexus letter from a physician — ideally one familiar with mental-health-related sexual dysfunction
  4. Personal statement (Form 21-4138) describing onset of ED, current severity, and functional impact
  5. Spouse statement (Form 21-10210) if applicable

“It is at least as likely as not that this veteran’s erectile dysfunction was caused or aggravated by his service-connected Post-Traumatic Stress Disorder and the medications (sertraline 100mg daily) used to treat it. The neurobiological effects of chronic PTSD on the autonomic nervous system, combined with the well-documented sexual side effects of SSRIs, provide a clear physiological pathway for this connection.”

Strategy

Over 20 years, SMC-K alone is worth approximately $33,500 in today’s dollars (before COLA). The nexus letter and filing cost approximately 4 hours of your time and possibly a few hundred dollars for an independent medical opinion. The return on time is staggering. This is the single highest-priority claim for any veteran with service-connected mental health and ED symptoms.
